Insurance & Payment

O'Brien Physical Therapy, LLC provides physical therapy on a “fee for service” basis. Payment will be accepted at the time of service. If you wish to submit a claim to your insurance company for reimbursement, you will be provided proof of payment, a "super bill" including necessary and appropriate codes as well as any supporting documentation requested by your insurance company.

Reimbursement is not guaranteed, but is most consistent with Out of Network provider rates. You may contact your insurance company prior to your appointment to determine whether Out of Network provider services are covered and at what rate.

Accepted forms of payment include: Credit/debit card, check or cash at the time of service. You may also use your Flexible Spending (FSA) or HSA. Learn more about insurance on our FAQ page.
